PIAA sports physicals will be held on June 12th from 9:00 am to 11:00 am and August 7th from 7:00 am to 10:00 am. Students planning to receive a physical on one of these dates must have a completed packet signed by both the student and parent/guardian. Physicians cannot complete a physical if the packets are not filled out and signed. If students are receiving physicals from their primary care physician or at urgent care, physicals must be on the PIAA form and completed after June 1st. School physical forms ARE NOT the same as the PIAA physical form. Valid PIAA physicals must be completed and turned in to Miss. Virginia to participate in a sport.

If completing the form on FinalForms, Section 5 must be printed, completed, and turned in at the time of the physical.
